For yourself
You already have a directive, a will, or a list of medications. The registry gives those documents a fixed address and lets you decide, file by file, who may retrieve them.
- Register in your own name and pay once per upload. There are no monthly or annual fees afterward.
- Upload the files you have now and add the rest later.
- Name up to five people and choose which documents each one may see.
- Grant hospitals and courts access to specific documents if you wish.
- Carry the printable ID card so someone can find your registry.
For a parent or loved one
Many registries are set up at a kitchen table with an adult child alongside. If you hold power of attorney or serve as trustee, you may register on their behalf.
- A registrant, trustee, attorney, or person with power of attorney may register.
- Gather their existing documents together. Nothing new has to be drafted.
- Add yourself as a designated person so you can retrieve the files when needed.
- A gift card lets you cover the cost while they keep control of the account.
For attorneys and trustees
A well-drafted estate plan is only useful if it surfaces at the right time. Registering a client's documents gives the people named in them a verified way to retrieve them.
- Register as the attorney or trustee for a client.
- Set document-specific permissions for family members, hospitals, and courts.
- Replacement uploads cost $50 when a document is amended.
- RegistryAZ stores existing documents. It does not draft them or give legal advice.
For hospitals and courts
If a patient or party has granted your institution access, you can retrieve the permitted documents through registry lookup. There is no charge for retrieval.
- Search with the registrant's name and one identifier.
- Complete the verification step.
- Receive a time-limited, encrypted link to the documents you were permitted to see.
- Every access attempt is logged for the registrant.
Retrieval is free for designated individuals, hospitals, and courts. You will only see the documents the registrant permitted for you.
