Setting up a registry is a short, practical process. Here is each step, what it asks of you, and what happens when someone needs a document.
What to gather
Start with the documents you already have. Nothing needs to be drafted, and nothing needs to be complete before you begin.

Your personal and contact details and an ID number
For each person you designate: their name, mobile number, and email
Optional: their relationship to you and extra contact details

Register
You can register for yourself, or as a trustee, attorney, or person holding power of attorney for someone else. Registration, purchase, and your account all happen in the RegistryAZ application.

Either way you pay once. There are no monthly or annual fees afterward.
Plans change. When a document is amended, upload the replacement for $50. Replacing an allergy or medication list is free. You can delete a document or change permissions at any time.
