Keep a say in your life, even when you can't speak.
RegistryAZ keeps the directives, will, and documents you have already prepared where the people you choose can retrieve them.
Pay once per document. No monthly or annual fees, ever.

A plan only helps if someone can find it.
Most people who prepare a directive or a will put it somewhere sensible: a drawer, a lawyer's office, a folder on a laptop. Then the moment arrives, and the people who need it are searching.
RegistryAZ exists so that your decisions reach the people you trust at the moment they are asked to act on them. That is the purpose. Everything else on this page is mechanics.
Why we do this
One place for the documents that carry your decisions.
Nothing new has to be written. Bring the documents you already have and give them a fixed address.

Personal information

- Insurance policy
- Emergency contacts

Up to 12 documents, each 5 MB or smaller, in PDF, JPG, JPEG, and PNG. These are examples of documents you already hold, not documents RegistryAZ creates.
Only the people you choose. Only the files you allow.
Access is decided document by document, so a hospital can see your directive without seeing your will.
Looking for someone's document? Registry lookupYou name the people
Designate zero to 5 individuals and decide which documents each one may retrieve. Change it whenever you like.
You include institutions if you wish
Give hospitals or courts permission for particular documents, and no others.
Retrieval is verified and logged
A lookup starts with your name and one identifier. After verification, the approved files arrive through a time-limited, encrypted link, and every attempt is recorded.
While you may grant access to hospitals and courts, there is no guarantee that hospitals or courts will utilize the service. A printable ID card helps others know your registry exists.

Questions people ask before they register.
Do I need all six documents ready to buy the bundle?
No. The six-upload bundle is available only when you first register, and you can upload your documents later. It costs $300 for six uploads.
Is there a monthly or annual fee?
No. You pay once for each upload and there is nothing more to pay to keep that document in your registry. There are no monthly fees, no annual fees, and no subscription. The only later costs are optional replacements and new uploads at the published prices.
How does someone retrieve a document?
A designated individual, hospital, or court can use registry lookup with your name and one identifier. After verification, they can retrieve only the documents you permitted. Links are encrypted and time-limited, and access attempts are logged. Retrieval is free.
Will a hospital know my documents are there?
You can grant a hospital access and carry a printable ID card to help others find your registry. There is no guarantee that a hospital or court will use the service.
Can I update a document after I upload it?
Yes. A replacement upload costs $50. Replacing an allergy or medication list is free; its original upload is subject to the normal upload price.
What if I cancel my account?
If you cancel within the first 30 days, you receive a 50% refund. No refund is available after 30 days. Once your account is cancelled, access ends and your documents are permanently deleted. See the terms of sale for complete conditions.
